The routine immunoassay for salivary cortisol seems to have better diagnostic performance than liquid chromatograph/tandem mass spectrometry, although measurement of normal salivary cortisone concentrations with the latter technique is very useful in identifying samples contaminated with topical hydrocortisone. Two LNSCs from each patient is recommended for routine screening, although one adequate saliva sample seems to perform well.

07.26.2013 -Update on Salivary Cortisol for the diagnosis of Cushing’s syndrome: methodological considerations Author: Raff, H. (2013) EndocrinePeriodic assessment of LNSC is extremely useful in monitoring patients for recurrence of Cushing's disease after pituitary surgery. With the large increase in the number LNSCs being ordered around the world, it is likely that more preanalytic and analytic issues will arise, which laboratorians and clinical chemists will need to resolve. Salimetrics salivary assay kits are expertly designed, developed and validated to ensure accuracy in saliva and proven to deliver precision results for saliva biomarkers. The Salimetrics Salivary Cortisol Enzyme Immunoassay Kit was specifically designed to standardize the detection of cortisol in saliva samples for research and biomedical laboratories. Using a small sample volume, this assay kit has an extended range that spans the expected cortisol levels found in human saliva. The average inter- and intra-assay precision coefficients of variation are low with no deleterious matrix effects often found in saliva which are characterized through dilution- and spike-recovery validation procedures. This cortisol assay kit has also been formatted to minimize cross reactivity for related steroids.Salivary cortisol levels are unaffected by salivary flow rate and are relatively resistant to degradation from enzymes or freeze-thaw cycles. Studies consistently report high correlations between serum and salivary cortisol, indicating that salivary cortisol levels reliably estimate serum cortisol levels. Cortisol production has a circadian rhythm, with levels peaking in the early morning and dropping to lowest values at night. However, levels rise independently of circadian rhythm in response to stress. Salimetrics Cortisol Diagnostic Salivary Immunoassay Kit (ELISA/EIA) (in vitro) Catalog number: 1-3102 (5-pack 1-3102-5) Format:96-well plate Type: Quantitative Method: ELISA/EIA Calibrator range: 0.012-3.000 ug/dL Sensitivity: <0.007 ug/dL"Late-night salivary cortisol (LNSC) is now considered the best approach to screen patients suspected of having endogenous hypercortisolism (Cushing's syndrome)"
